Red yeast rice (RYR), a fermented product of rice kernels with the red-pigmented mould Monascus purpureus has been used in Traditional Chinese Medicine (TCM) to treat blood circulation stasis, indigestion, and limb weakness in East Asian countries.1 RYR contains, among others, monacolins, pigments, organic acids, sterols (β-itosterol, campesterol, stigmasterol, sapogenins) and flavonoids.1,2 The most prominent ingredient of RYR is monacolin K, also known as mevinolin.3 Additionally, RYR contains several pigments, including the homologues rubropunctatin & monascorubrin (orange–red) as well as their reduced forms monascin & ankaflavin (yellow).4

In recent years RYR extracts have become a popular dietary supplement5 due to their various beneficial, e.g., antihyperlipidemic, anti-diabetic and anti-hypertensive activities.2
 
A safety assessment of RYR by the European Food Safety Authority (EFSA) was published in 2018.6 EFSA concluded that intake of monacolins from RYR as food supplements should not exceed 10 mg/day due to significant safety concerns. Besides monacolin K, the USP Dietary Supplement Verification Program is currently examining the yellow pigments (monascin & ankaflavin).[7]
